Job Purpose
To ensure the highest standards of care are delivered to clients by supervising, supporting, and mentoring care staff in the field. The Field Care Supervisor / Team Leader is responsible for monitoring care quality, conducting spot checks and assessments, supporting new staff, and acting as a key link between the office, care staff, and clients.
Key Responsibilities
1. Supervision & Support of Care Staff
Conduct regular spot checks, supervisions, and competency assessments of care staff in the field.
Mentor, support, and guide care staff, providing feedback and identifying training needs.
Support the induction and shadowing of new care staff, ensuring they are competent and confident before working independently.
Address performance or conduct issues promptly and escalate where necessary.
2. Quality Assurance & Compliance
Ensure care is delivered in line with individual care plans, company policies, and regulatory standards (e.g., CQC).
Complete quality monitoring visits and audits, reporting findings to management.
Ensure accurate and timely completion of care records, MAR charts, and other documentation.
Participate in investigations of complaints, incidents, and safeguarding concerns.
3. Client Assessments & Reviews
Conduct initial and ongoing assessments of clients’ needs, risks, and preferences.
Review and update care plans and risk assessments in consultation with clients, families, and professionals.
Respond to changes in clients’ needs, ensuring care packages are adjusted appropriately.
4. Communication & Liaison
Act as a key point of contact for clients, families, and professionals regarding care delivery.
Communicate effectively with office staff, care workers, and management to ensure continuity and consistency of care.
Attend and contribute to team meetings, handovers, and reviews.
5. Training & Development
Identify training needs and support the ongoing development of care staff.
Deliver or facilitate training sessions and competency checks as required.
Promote a culture of continuous improvement and learning.
6. Health, Safety & Risk Management
Promote and maintain safe working practices in line with health and safety policies.
Ensure staff follow infection control procedures and use PPE correctly.
Report and record accidents, incidents, and near misses, implementing corrective actions.
7. Administrative Duties
Maintain accurate and up-to-date records of visits, assessments, and supervisions.
Support rota management and ensure adequate staffing levels in the field.
Assist with on-call duties as required.
Person Specification
Essential Qualifications & Experience
NVQ/QCF Level 3 in Health & Social Care (or working towards)
Experience in a care setting, with supervisory or mentoring responsibilities
Knowledge of care sector regulations and best practice (e.g., CQC standards)
Experience in conducting assessments and risk management
